Flu Shots:  Marion County Public Health will be administering the influenza vaccine at school on Thursday, October 15. For your child to participate, we must have this signed consent form returned by October 1st.

• If your child is on Medicaid (Title XIX) or has no insurance coverage for immunizations, the flu vacc. will be given to your child at no cost (with the signed consent form).
• If your child has insurance coverage that pays for immunizations, the cost for the flu shot is $15.00. To have your child vaccinated, please fill out the consent form and return it with a check/money order in the amount of $15.00 to your child’s school.
